Your Information. Your Rights. Our Responsibilities.

This Notice of Privacy Practices describes how your medical information may be used and disclosed and how you can access this information. Please review it carefully.

 

1. Your Rights

1.1 Get an electronic or paper copy of your medical record
You have the right to request access to your medical record. We will provide you with a copy or a summary within 30 days, with a reasonable cost-based fee, if applicable.

1.2 Request that we correct your medical record
If you believe your health information is inaccurate or incomplete, you may request corrections. We will respond in writing within 60 days, explaining any denial.

1.3 Request confidential communications
You can specify how you want us to communicate with you or how to send correspondence. We will accommodate reasonable requests.

1.4 Ask us to limit what we use or share
You may request limitations on the use or sharing of certain health information. We may deny the request if it affects your care, but we will comply whenever possible.

1.5 Get a list of those with whom we have shared your information
You can request a list of disclosures, excluding treatment, payment, and healthcare operations. One free report per year; additional requests within 12 months incur a reasonable cost-based fee.

1.6 Get a copy of this privacy notice
Request a paper copy of this notice at any time, even if you agreed to receive it electronically.

1.7 Choose someone to act on your behalf
Authorized individuals, such as a medical power of attorney or legal guardians, can exercise your rights on your behalf.

1.8 File a complaint
If you feel your rights have been violated, contact our Privacy Officer at 200 Frandorson Cir, Apollo Beach, FL 33579, or email support@eonmedicalcenter.com. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.

 

2. Your Choices

You can express your preferences on how your health information is shared in specific situations:

2.1 Share information with family, friends, or others involved in your care
2.2 Share information in a disaster relief situation
2.3 Include your information in a hospital directory

In emergencies or when you are unable to communicate, we may use our professional judgment.

Exceptions: We never share your information without your written permission for marketing, selling your information, or most psychotherapy note sharing. You may opt out of fundraising contacts.

 

3. Our Uses and Disclosures

EON Medical + Wellness generally uses or shares your health information in the following ways:

Treat you: We may use your health information and share it with other professionals treating you.
Example: A doctor treating you for an injury asks another doctor about your overall health condition.

Run our organization: We may use and share your health information to manage our practice, improve your care, and contact you when necessary.
Example: We use health information about you to manage your treatment and services.

Bill for your services: We may use and share your health information to bill and receive payment from health plans or other entities.
Example: We provide information about you to your health insurance plan so it will pay for your services.

 

4. Other Uses and Disclosures

EON Medical + Wellness may use or share your health information in ways that generally contribute to the public good, such as public health and research. Help with public health and safety issues: We may share your health information in certain situations, such as preventing disease, helping with product recalls, reporting adverse reactions to medications, reporting suspected abuse, neglect, or domestic violence, and preventing or reducing a serious and imminent threat to anyone's health or safety.

Do research: We can use or share your information for health research.

Comply with the law: We will share information about you if state or federal laws require it, including with the U.S. Department of Health and Human Services to ensure we are following federal privacy laws.

Respond to organ and tissue donation requests: We can share health information about you with organ procurement organizations.

Work with a medical examiner or funeral director: We can share health information with a coroner, medical examiner, or funeral director when an individual dies.

Address workers’ compensation, law enforcement, and other government requests: We can use or share health information about you for workers' compensation claims, for law enforcement purposes or with a law enforcement official, with health oversight agencies for activities authorized by law, and for special government functions such as military, national security, and presidential protective services.

Respond to lawsuits and legal actions: We can share health information about you in response to a court or administrative order, or in response to a subpoena.

EON Medical + Wellness never markets or sells personal information.

 

5. Our Responsibilities

EON Medical + Wellness is required by law to maintain the privacy and security of your protected health information. We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information. We must follow the duties and privacy practices described in this notice and give you a copy of it. We will not use or share your information other than as described here unless you tell us in writing. If you give us permission, you may change your mind at any time. Let us know in writing if you change your mind.

6. Changes to the Terms of This Notice

EON Medical + Wellness may change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our website.

 

7. Effective Date of This Notice

This Notice of Privacy Practices is effective as of December 2, 2023, and will remain in effect until a new Notice of Privacy Practices is approved and posted.
